Southern Jamaica Plain Health Center (SJPHC) is one of two community health centers licensed by Brigham and Women’s Hospital.
Southern Jamaica Plain Health Center (SJPHC) is one of two community health centers licensed by Brigham and Women’s Hospital. The mission of SJPHC to provide personal, quality health care with respect and compassion to our diverse community. Every year, more than 10,000 patients in Jamaica Plain and surrounding communities make over 45,000 visits to SJPHC. Bilingual staff provide patients with comprehensive, high quality primary care. SJPHC was started in the early 1970's by a group of Jamaica Plain (JP) mothers looking for neighborhood health care for their families. SJPHC becgan to come together through volunteers, doctors and nurses who were committed to the southern Jamaica Plain. Since it was addressing a community need, SJPHC expeirenced very rapid growth in 1974 moved into a storefront on Centre Street. After moving, SJPHC began serving families in Adult Medicine, Pediatrics, Women's Health and Mental Health Services. In addition to these core medical services, SJPHC has a WIC office, connects patients with services such as food assitance, domestic violence assistance and tobacco cessation support. By leveraging local community partnerships, we focus our preventative activities around issues important to our contituents, including youth violence, childhood obesity and asthma. In addition to preventive and clinical interventions, SJPHC has taken social action against inequity in health. In 2010, a partnership led by SJPHC was the only community disparities partnership funded in the city, and will be a model for Boston of how neighborhoods address the social determinants of health.
